Gout attacks usually last 1 to 2 weeks untreated, but the good news is that effective treatments can provide significant pain relief within 24–48 hours when started promptly.
Gout is caused by excess uric acid in the blood, which leads to the formation of uric acid crystals in your joints and soft tissues. These crystals trigger sudden, severe inflammation – and without treatment, the pain and swelling can persist for days. Why Gout Treatment Works
Modern gout treatment is built on decades of clinical evidence and well-understood mechanisms. When too much uric acid accumulates in the blood (a condition called hyperuricemia), it forms monosodium urate crystals that deposit in joints, triggering the inflammatory response responsible for gout flare ups. Treatment works by targeting this process at two levels:
Acute relief medications (NSAIDs, colchicine, corticosteroids) act directly on the inflammation, reducing pain and joint swelling within hours to days.
Ultrasound Guided Cortisone Injections reduce inflammation and are injected directly into the affected joint, often providing pain relief much quicker.
- Urate-lowering therapy (allopurinol, febuxostat, uricosurics) reduces the amount of uric acid your body produces or increases how much it excretes, gradually dissolving existing urate crystals and preventing new ones from forming.
The evidence behind these approaches is substantial. In the NOR-Gout study, patients who followed a treat-to-target strategy saw their mean serum uric acid drop from approximately 500 to 337 μmol/L over five years. Ultrasound imaging confirmed that crystal deposits dissolved in 83.4% of cases, and tophi visible nodules of aggregated crystals called tophi resolved in 63.2%.
Data from the CARES trial showed that remission rates (defined as no flares, consistently low uric acid levels, and no tophi) rose from 37.4% at year one to 63.1% by year six. These are not theoretical numbers, they represent many patients returning to normal life, free from the disruption of recurrent attacks.
Multiple treatment options exist for different patient needs, including those with kidney disease, high blood pressure, diabetes, or other medical conditions that may limit which medications are suitable.

Treatment for gout divides into three complementary approaches: managing the acute attack, preventing future attacks with long term medication, and supporting both through lifestyle changes. The right combination depends on your specific symptoms, how frequently attacks occur, and any other health conditions you have.
Acute Attack Treatment
Immediate treatment for gout flares aims to reduce inflammation and pain quickly. The main options are:
For gout flares affecting a single joint, cortisone injections for gout delivered under ultrasound guidance offer improved accuracy and faster relief. This approach is especially valuable for small or deep joints where blind injection risks missing the affected area.
Corticosteroids can be administered if NSAIDs or colchicine are unsuitable. Oral prednisolone (20–35 mg daily for 3–5 days) manages polyarticular flares, while a corticosteroid injection can be injected directly into the affected joint for mono-articular attacks. Corticosteroid injections can provide rapid pain relief during attacks and are particularly useful when other medications are contraindicated. Ice packs can be applied for up to 20 minutes to relieve pain alongside any medication approach.
NSAIDs are often the first line of defence in treating gout flares. NSAIDs like ibuprofen are commonly used for gout pain relief, with naproxen 500 mg twice daily being a typical prescription. These work best when started within 12–24 hours of symptom onset. NSAIDs are contraindicated in patients with kidney disease, peptic ulcer disease, or uncontrolled high blood pressure. Where gastrointestinal risk is present, a proton pump inhibitor may be co-prescribed.
Long Term Prevention Treatment
Urate-lowering therapy is recommended when patients experience two or more gout attacks per year, have tophi, show signs of joint damage, have kidney disease, or have a history of kidney stones. The goal is to lower uric acid levels below 360 μmol/L (or below 300 μmol/L in patients with tophi or severe disease).
Allopurinol – allopurinol is a common uric acid-lowering medication for gout. It is a xanthine oxidase inhibitor that reduces uric acid production. Treatment typically begins at a starting dose of 100 mg daily, with gradual increases every four weeks until the target uric acid level is reached. Maintenance doses can reach up to 900 mg daily depending on tolerability and kidney function.
Febuxostat – febuxostat was FDA approved in 2009 for treating hyperuricemia in gout. It is prescribed for patients who are intolerant of allopurinol or have not responded adequately. The usual dose is 80–120 mg daily. Febuxostat is not recommended for patients with significant ischaemic heart disease or congestive heart failure.
Regular blood tests are necessary to monitor uric acid levels during treatment, typically monthly at first and then every three to six months once the target is reached.
THE PROCESS
How Gout Treatment Works
Treating gout involves managing acute flares and lowering uric acid levels long-term. Here is what the process typically looks like from first consultation to ongoing management.
Initial Assessment and Diagnosis
Your doctor will take a full medical history, including the pattern and location of joint pain, family history of gout, current medications, and any existing health conditions. Gout is more common in men as they age, and risk factors including obesity, kidney disease, and diuretic use will be evaluated.
Diagnostic steps include:
Blood tests – to check uric acid levels, kidney function, liver enzymes, and full blood count. Note that serum uric acid may be normal during an acute attack and should be rechecked after the flare resolves
Joint aspiration – fluid is drawn from the affected joint to examine for urate crystals under polarised microscopy and to exclude septic arthritis
Imaging – plain X-rays detect joint damage in chronic gout, while ultrasound or Dual Energy CT (DECT) scans can identify crystal deposition in joints and soft tissues even between attacks
An accurate diagnosis is essential because other conditions, including septic arthritis and pseudogout, can produce similar common symptoms of inflammation and joint swelling.
Based on your individual risk factors and comorbidities, your doctor will prescribe the most appropriate acute medication. NSAIDs like ibuprofen are commonly used for gout attacks as first-line treatment. For patients with contraindications, colchicine or corticosteroids offer alternatives.
You will receive instructions on managing the current flare: rest the affected joint, elevate it, apply ice for up to 20 minutes at a time, and avoid other medications that could worsen symptoms. Pain typically starts decreasing within 24–48 hours of starting treatment, with full resolution expected within one to two weeks.
Once the acute flare has settled (usually one to two weeks), your doctor will discuss whether urate lowering therapy is appropriate. If you meet the criteria, frequent attacks, tophi, certain medical conditions like kidney disease, or joint damage, a treatment plan will be developed.
This involves:
Starting allopurinol or febuxostat at a low dose with gradual titration upward
Regular monitoring of uric acid levels to confirm target is being reached
Prophylactic medication to prevent flares during the titration period
Ongoing lifestyle modification guidance and support
Periodic review to assess for tophi resolution, joint damage, and overall progress
THE EVIDENCE
Treatment Success Rates
The evidence for gout treatment effectiveness is strong across all approaches:
Acute attack relief – there is no clear superiority between NSAIDs and colchicine for treating acute gout attacks; both provide rapid relief when started promptly. Pain relief begins within 12–24 hours, with meaningful reductions in pain and inflammation within 24–48 hours. Full resolution typically occurs within 3–10 days according to UK clinical guidelines.
Cortisone injections – corticosteroid injections can relieve inflammation during gout attacks affecting a single joint. Cortisone injections can be administered three to four times a year when needed, providing targeted relief with minimal systemic effects. Ultrasound guidance achieves technical accuracy of approximately 95% compared to around 82% for landmark guided injections, reducing the chance of missing the affected area.
Long term urate lowering therapy – in the NOR-Gout study, 71.2% of patients achieved their target uric acid level below 360 μmol/L over five years. Only 16% of patients experienced any gout flare in the final year of follow-up, a dramatic reduction compared to baseline. In the CARES trial, remission rates reached 63.1% by year six, with over 59% of patients achieving remission at least once during the study period.
Joint damage prevention – ultrasound imaging in the NOR-Gout study demonstrated resolution of crystal deposits (double contour sign) in 83.4% of patients and dissolution of tophi in 63.2%, confirming that sustained treatment can reverse, not just halt, the structural damage of chronic gout.
These outcomes depend heavily on adherence. With overall ULT adherence estimated at only 47% across studies, the patients who achieve the best results are those who continue their medication consistently and attend regular monitoring appointments.

Why Ultrasound Guided Injections Are Better for Treating Gout Flare Ups
When a gout flare strikes a single joint, commonly the big toe, ankle, knee, or other joints, a steroid injection delivered directly to the affected area offers targeted, reducing inflammation precisely where it is needed. The question is whether ultrasound guidance makes a meaningful difference compared to standard landmark guided technique.
The evidence says yes, particularly for accuracy. A 2025 systematic review of intra-articular injections found that ultrasound guided procedures achieved technical accuracy in the mid 90% range, compared to approximately 82% for landmark guided injections. This matters because an injection placed outside the joint space will not deliver the same pain relief, and in gout, where the affected joint is often severely swollen and difficult to palpate, precision is especially important.
Benefits of ultrasound guidance include:
Higher accuracy – the needle is visualised in real time, ensuring medication reaches the joint space
Better suitability for small or deep joints – gout frequently affects the first metatarsophalangeal joint (big toe), midfoot, and ankle, where blind injection is less reliable
Reduced risk of complications – fewer extra articular injections mean less chance of steroid depositing in surrounding soft tissues or blood vessels
More consistent pain relief – studies show more reliable short term improvements in pain and function
It is worth noting that long term outcome differences between ultrasound guided and landmark guided injections are less well established, and the equipment and trained personnel required do increase cost. However, for patients experiencing an acute gout flare who need dependable, fast pain relief from a corticosteroid injection, ultrasound guidance provides a meaningful advantage.

Colchicine commonly causes diarrhoea and nausea, and in rare cases, bone marrow suppression at higher doses – especially in patients with impaired renal function. Corticosteroids may cause mood changes, elevated blood sugar, and increased infection risk. Allopurinol can rarely cause hypersensitivity reactions, and febuxostat carries cardiovascular warnings for certain patients. Uricosurics increase the risk of kidney stones. Your healthcare professional will select medications based on your individual risk factors and monitor for side effects through regular blood tests.
Can gout be cured completely?
Gout is a chronic condition, but it is highly treatable to the point of remission. Remission means no gout attacks, consistently low uric acid levels, and no tophi. The CARES trial demonstrated that approximately 63% of patients on urate-lowering therapy achieved remission by year six. With sustained treatment and adherence, many patients experience complete freedom from symptoms of gout – though stopping medication typically allows uric acid to rise again and attacks to return. Long term management is the key to lasting control.

I left feeling much more informed about my situation and after following his instructions my symptoms have improved massively.
