Physiotherapy, oral medication, rest and lifestyle changes may have offered little lasting improvement. Meanwhile, the prospect of lumbar spine surgery, with its lengthy rehabilitation, surgical risks and uncertain outcomes, can feel overwhelming.
Caudal epidural injections were developed for exactly this situation. As a minimally invasive epidural steroid injection, the procedure bridges the gap between conservative care and surgery by delivering corticosteroid medication and a local anaesthetic close to the inflamed nerve roots responsible for your pain. Rather than treating your whole system with medication, it targets the source of inflammation in the epidural space.
This is a well-established approach. Epidural steroid injections are among the most widely studied interventional pain procedures available, and are commonly used to manage radicular pain, sciatica, spinal stenosis and other conditions involving spinal nerve irritation. The published evidence consistently shows that many patients experience worthwhile pain relief, with benefits typically lasting from several weeks to several months.
Why Caudal Epidural Injections Work
The caudal approach reaches the epidural space through the sacral hiatus, a natural opening at the base of the spine near the tailbone. Once the medication is delivered, the corticosteroid works to reduce inflammation and swelling around irritated spinal nerve roots, while the local anaesthetic provides more immediate relief. Here’s why this mechanism is effective:
The steroid is placed close to where nerve-root inflammation occurs, reducing the swelling that causes radiating leg pain and lower back pain. Because the medication is delivered locally rather than taken by mouth, there is less reliance on high-dose systemic drugs such as oral steroids or opioids, and fewer whole-body side effects, which is particularly valuable if you manage other health conditions or can’t tolerate certain medications.
Unlike some other epidural techniques, the caudal approach can reach several nerve-root levels with a single injection. The larger volume spreads upward through the spinal canal, which makes it well suited to more diffuse lower-back problems or symptoms affecting both legs.
Because the caudal entry point sits below and away from the dural sac, the risk of dural puncture and the headache that can follow is lower than with approaches higher up the spine. The caudal route takes full advantage of this.
The procedure is performed as an outpatient treatment, with no incisions, no general anaesthetic and no hospital stay. Most patients return to normal activities within a few days.
Rather than masking pain throughout your body, a caudal epidural injection is designed to settle the specific nerves causing your symptoms, aiming for relief at the source.
CONDITIONS
What Conditions Do Caudal Epidural Injections Treat?
Caudal epidural injections are used for a range of conditions linked to spinal nerve irritation in the lower back. A few of the conditions that can be treated with these injections include the following:
Sciatica and lumbar radiculopathy:
Sciatica and lumbar radiculopathy are the most common reason for the procedure. Radicular pain caused by a herniated or bulging lumbar disc pressing on a spinal nerve, frequently at the L5-S1 level, often responds well to anti-inflammatory medication delivered to the affected nerve roots.
L4-L5 and L5-S1 disc bulges and herniations:
If you’ve had an MRI reporting a disc bulge, protrusion or herniation at L4-L5 or L5-S1, these are the levels most commonly involved in sciatica, and they’re where a caudal epidural is most often used. It’s worth being clear that a disc bulge on a scan doesn’t always cause symptoms, and not everyone who has one needs an injection. Where the bulge is genuinely irritating a nerve root and causing leg pain, numbness or weakness, a caudal epidural can help settle that inflammation. Your assessment is what decides whether it’s the right option for you.
Spinal stenosis:
Narrowing of the spinal canal can cause neurogenic claudication (pain, heaviness or weakness in the legs on walking that eases with rest). A caudal epidural can help some patients walk further with less discomfort, though longer-term relief may require repeat treatment.
Discogenic lower back pain:
Chronic low back pain arising from damaged discs, even without significant nerve compression, may benefit in some cases. The evidence here is more modest, and we’ll be honest with you about what to expect.
Pain after previous back surgery:
After a surgery, where scar tissue and altered anatomy make other approaches more difficult, the caudal route is often the safer option.
Who it isn't suitable for:
A caudal epidural isn’t right for everyone. Active infection, bleeding disorders or blood-thinning medication, allergy to corticosteroid or local anaesthetic, and poorly controlled diabetes are among the reasons the procedure may not be appropriate. These are all things your doctor will assess with you beforehand.
THE PROCEDURE
How the Procedure Works At My MSK Clinic
Having a caudal epidural injection is a straightforward, structured process at My MSK Clinic in Manchester. The procedure itself usually takes around 15 to 30 minutes, and you’ll go home the same day.
- Step 1: Consultation and preparation
Your appointment begins with a consultation with a GMC-registered doctor, who will review your symptoms, medical history and previous treatments. A physical examination helps confirm the diagnosis, and any imaging you’ve had (MRI or CT) is reviewed to pinpoint the source of your pain. Your doctor will also check for anything that affects suitability, bleeding risk, allergies, and blood-sugar control if you have diabetes. In the procedure room, you’ll lie comfortably face down. The skin over the sacral area is cleaned with antiseptic, and a local anaesthetic is used to numb the site so the injection itself is as comfortable as possible.
- Step 2: Ultrasound-guided injection
At My MSK Clinic, every caudal epidural is performed under real-time ultrasound guidance. Using ultrasound, your doctor guides a fine needle through the sacral hiatus into the caudal epidural space, watching placement live as it happens, with no radiation exposure, unlike fluoroscopy (X-ray) guided injections used in many hospital settings. The corticosteroid and local anaesthetic mixture is then delivered into the epidural space, so the medication reaches the intended nerve roots.
- Step 3: Recovery and results
Afterwards, you’ll be monitored for a short period and your doctor will check you can walk comfortably before you leave. Some temporary numbness, heaviness or mild weakness in the legs is normal as the local anaesthetic works, and this settles quickly. The local anaesthetic may ease your pain almost immediately, but the anti-inflammatory effect of the steroid builds gradually over the following days to weeks. We’ll arrange follow-up, typically at around three weeks and again at three months, to review how well the injection has worked and discuss next steps if needed.
What Makes the Caudal Approach Different
Not all lumbar epidural injections are the same, and the caudal approach has some distinct advantages over other techniques.
Compared with transforaminal and interlaminar epidural injections, the caudal route enters from below through the sacral hiatus, rather than between the vertebral laminae or through the neural foramina. That single entry point can treat several nerve levels at once, which makes it a good option for widespread lower-back symptoms or pain affecting both legs.
The risk profile is favourable, too. The caudal approach carries a lower chance of dural puncture, and it avoids the risk of inadvertent injection into the small arteries near the neural foramina, a concern more associated with transforaminal injections. Serious neurological complications from a properly performed caudal epidural are very rare.

How quickly will I feel pain relief after the injection?
The local anaesthetic may give you some immediate numbness and relief within minutes. The corticosteroid, which provides the longer-lasting benefit, usually takes several days to start working, with the fuller effect often reached within two to three weeks.Â
What are the potential risks and side effects?
Most side effects are temporary: soreness at the injection site, mild discomfort, short-lived numbness or weakness in the legs, and a temporary rise in blood sugar in patients with diabetes. A headache can occur if the covering of the spinal cord is inadvertently punctured, though this is low-risk with the caudal approach. Serious complications are extremely rare. Bleeding and infection at the site are possible but uncommon when proper sterile technique is followed, which it always is.
How many injections can I have in a year?
Typically treatment is limited to around two to three epidural steroid injections per year, with adequate spacing between them. This helps us assess how well each injection works and avoids the potential effects of too much steroid. Your doctor will advise what’s right for you.
Will I need someone to drive me home?
It’s sensible to arrange for someone to drive you home, as the local anaesthetic can cause temporary numbness or weakness in your legs. We’d recommend avoiding driving for the rest of the day.
Is the Caudal Epidural Injection painful?
You may feel a brief pinch or pressure during the injection. We use a local anaesthetic to keep discomfort minimal, and many patients describe it as less painful than expected.
When can I return to work and normal activities?
Downtime is usually minimal. Many patients return to light activities the same or next day. We’d suggest avoiding strenuous activity, heavy lifting and vigorous exercise for about a week. When you return to full work duties depends on how physical your job is and how you’re feeling, for most people it’s within a few days to a week.
What if the injection doesn't give me enough relief?
If you don’t get enough relief, there are options. Your doctor may suggest a repeat injection, consider an alternative approach such as a transforaminal or interlaminar route, or review your imaging to reconfirm the diagnosis. Combining injections with a structured rehabilitation and physiotherapy programme often improves results, and if injections aren’t enough, we can discuss onward referral for a surgical opinion.

I left feeling much more informed about my situation and after following his instructions my symptoms have improved massively.
